The short answer: get the form from the office that issues it. The longer answer is why hosting copies here would make things worse rather than better.

We do not host copies of government forms, deliberately. A downloaded PDF of a barangay clearance form goes out of date the moment the office revises it, and someone arriving with last year’s form has wasted a trip. Worse, a form hosted on a third-party site is exactly what a phishing attempt looks like. We link to the issuing office instead.

Civil registry documents

Birth, marriage and death certificates come from the Local Civil Registry Office, and PSA-authenticated copies come from PSA. Which one you need depends on who is asking. Certificates and vital records.

Business permits

Application forms are issued by your LGU’s Business Permits and Licensing Office and they vary. Business permits and licences.

Social welfare IDs

Senior citizen, PWD and solo parent IDs are processed by the City or Municipal Social Welfare and Development Office. Social services.

National agency forms

SSS, PhilHealth, Pag-IBIG, DFA and LTO publish their own current forms on their own sites. Those are the ones to use, and they are not your city hall. All services.

What we will build insteadNot a form repository. A per-LGU page that says which office issues which form, where that office is, and what the current requirement list is according to that LGU’s own Citizens Charter. That is more useful than a folder of PDFs and it does not go stale silently, because the charter carries its own date.
