Citizen reporting
This is for a specific DPWH contract: a project that is not where it says it is, was never built, or fell apart within a year. For a broken streetlight, a hanging wire or anything else in your barangay, use the community report form instead.
Every project in the dataset has a contract ID, a short code like 23CN0127. Search for the project on Bisto Proyekto, copy the contract ID, and include it. A report with a contract ID is a record. A report without one is an anecdote, and it is much harder to do anything with.

What to report
Pick the one that fits. If none of them do, the last one is not a filler category, it is a real option.
The contract says completed. You have walked the location and there is nothing built. This is the report that matters most and the hardest one to make stick, so bring the contract ID and a photo with the location visible.
The work exists but not where the contract says. Common with drainage and road segments described by station number rather than barangay.
Completed within the last few years and already cracked, collapsed, silted or washed out. Note the completion date from the contract.
Marked completed but the work stops halfway. Photograph where it stops.
A flood control project was built and the same street still floods. This is not necessarily a failure of the contract, but it is worth recording, and if a drainage outfall was never connected, that is.
